First when you are flying if you enter a dungeon from the outside while still flying, provided you don't get a loading screen, and manage to keep flying for a good 5+ seconds you take damage as if you fell from a height, even s doesn't stop this. This can kill you if you fly long enough. Second I've noticed my character stop running and flying sometimes for no reason, Im just sitting there holding down w and i fall out of the sky or go back into walking animation with plenty of stamina left.
