Epicenter not healing 10% hp on hit?

I was noticing that it seems like I am losing a lot of HP in comparison to when I play Madness. Then I noticed that every time I use epicenter, my HP does not seem to recover at all. The ability says it should recover 10% hp on hit but it looks like this is not happening for some reason ... anyone else experiencing this issue?
