Upgrading weapon/Accessories

This may sound noobish but I have upgraded both weapons and accessories just fine I'm just not sure what the terms mean (Replaced/Successful/Very Successful) could someone explain them?

When using gems or other weapons to upgrade, you will notice a green line showing different upgrade stages at the top of the window. When you place a weapon /accessory or gem into the slot, you will see the line go up and also see a "min" and "max." If you are very successful in the upgrade, the line will fill to max, whereas successful means it only filled to the "min" line. I have never seen replace, but I think replace means that the upgrade failed and that you need to replace the item you used to try to upgrade your weapon with another one.
