Skill Delay, Monster Freezes

Basically, I have noticed huge skill/combat delays and monster freezes. At times, the monster will suddenly teleport or their AOE attack ( 16m for that specific monster ) would hit me when I'm over 20m away from it.

Another thing I noticed is that when I use my rotations ( RB, LB, F, RB, LB, 1, etc. ), the delay was HUGE. I would see the F button light up, ready to use, but when I click it, my RB and LB and other buttons that I clicked before it would still be in the process of being used. Not a 1 second delay, but over 4 - 5 seconds.

Btw, this started happening in the middle of doing soulstone plains and continued happening through dungeons that I tried to ( failed :< ) do. I've tried logging off, and restarting the game.
