Cobalt Captain and other costume issues

I noticed someone wearing the Cobalt Captain costume earlier, and another person wearing a costume that didn't even show up in their profile (the costume slot was empty for some reason). I was wondering 1) how is that Cobalt Captain costume even obtainable, and 2) if anyone else has noticed weird costume-related issues.
