Portal/Teleportion disconnection

(Sorry for reposting, I figured this may be more appropriate in this section)

 

I've been getting disconnected every time I attempt to portal out of a dungeon, between locations, and through teleporting. The loading screen will show up, then the game proceeds to "not respond", then the loading appears to start and half way the game disconnects. I may include a screen shot of this if that may be helpful.  

 

Does anyone else experience this particular issue? Is there anything I can do to remedy this within the ToS?
