Dodging Boss attacks, that home in, What is the trick to avoiding it?

Topic says it all i'm getting into bosses now have, attacks that follow you have a huge hit box, when they get close, and seem to stun, paralyze or otherwise disable you. Seems easy " just dodge it " is all i ever read, but from my experience. Those kinds of attacks feel unavoidable, like you are meant to tank it, back step? you'd have to be psychic to avoid them with back step, defense break? sure but those have cool downs.

 

So i am curious from the vets how to you avoid some of these seemingly unavoidable attacks...

 

Edit: specifically second boss in the tomb dungeon, he spams charge/grab and it says to dodge it but i don't understand how,  even if i am clear across the room, he charges toward me changing direction based on movement so he literally homes in on my position...
