Can't Use Gamepad

I hook up my controller but when I click the "Gamepad Mode" option it says "Your controller has been disconnected" message. 
I'm sure this has been posted many times before but, I can't seem to find a search option in these forums. Only a filter that.....actually sucks.

I was getting the same message when i plugged in my Xbox one controller. Then I tried the Xbox 360 controller and it worked.

problem im having is multiple skills bound to same key and will not let me change the skill bind

FWIW, I experienced the same error with my Steam controller. My solution was to run Steam as admin.  YMMV
